The Consent Form
Tattoo Studio at Camp VC 2026
Everyone who books completes a consent and medical history form before their appointment. You'll receive your personal link by email after booking - it takes about 5 minutes. This page shows everything the form asks, so you know exactly what you're signing up to before you book.
Your details
- Full legal name, as on your ID.
- The email address you booked with.
- Date of birth - you must be 18 or over.
- A contact phone number and your postal address.
- An emergency contact - a name and phone number.
- Where the tattoo will go (arm or leg - not head or neck). You can confirm or change this with your artist on the day.
Photo ID
You'll upload a photo of your passport or driving licence. It's stored securely and visible to studio staff only. You'll also show the same ID in person on the day.
Health questions
Answer honestly - your artist will review these before the appointment. A yes doesn't automatically cancel your booking: some answers just mean a chat first, but the exclusions listed at booking (pregnancy or breastfeeding, bleeding disorders or blood thinners, immune conditions or immunosuppressants, skin conditions at the tattoo site, allergies to inks or latex, under 18) mean we can't tattoo you and the deposit is not refunded.
- Any infection or illness today (including a cough, cold, or feeling under the weather)?
- Currently pregnant or breastfeeding?
- Haemophilia, bleeding or clotting disorder, or currently on blood thinners?
- Skin condition at or near the area to be tattooed (eczema, psoriasis, moles, sunburn)?
- Allergies, including to inks, latex, metals, or plasters? (details asked if yes)
- Diabetes?
- Heart condition?
- Immune system condition, or currently taking immunosuppressant medication? (details asked if yes)
- Epilepsy, tendency to faint, or seizures?
- Taking any medication? (details asked if yes)
What you sign
You type your name and draw your signature to authorise this statement:
I confirm the information above is accurate and complete. I will not attend my appointment under the influence of alcohol or drugs. I understand a tattoo is permanent, that healing and final appearance vary from person to person, and that tattooing carries risks including infection, allergic reaction, scarring and swelling, which can occur even when the artist takes all proper care. I understand my tattoo artist may decline to proceed based on my answers or on their assessment when I arrive. I have read and understood the aftercare protocol.
You also tick to confirm: I will not attend my appointment under the influence of alcohol or drugs.
And a separate tick to consent to us processing your health answers so we can tattoo you safely - kept in line with the privacy notice, then securely deleted. You can withdraw it by emailing the studio.
